FEEDBACK AND COMPLAINT

If you have a concern about your current supports or services, it’s important to talk about it. 

Anyone receiving NDIS supports or services may provide feedback via Contact Us.

 

To make a complaint you can do so in writing or verbally.

 

To do this in writing, please download the form and return this via email.

This will help identify an issue to improve the quality of your supports provided in your package.

 

  • Your complaint will be handled with confidentiality and respect 

  • Advise you on how long the complaint will take

  • Investigate the complaint fairly 

  • Provide a thorough response detailing the outcomes.

  • Protect your information and privacy
